Please refer to the FAQ section of the new \u2018Application for Case Study Registration\u2019 document for more information regarding its use.<\/td><\/tr><tr><td>HIPAA Authorization Template for Case Study<\/td><td>This modifiable template can be used when the covered entity does not have a specific case study HIPAA authorization form for use.<\/td><\/tr><tr><td>Consent Form Template for Case Study<\/td><td>This modifiable template can be used when the covered entity does not have a specific case study consent form for use.<\/td><\/tr><tr><td>Notification Form: Participation in a Research Study Approved or Exempted by an External IRB<\/td><td>This new form must be submitted by any UNE-affiliated faculty, staff, or student who are (or will be) engaging in research activities for a study that was approved or exempted by an external IRB. Please refer to the FAQ section of the notification form for more information regarding its use.<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<p>On an on-going basis, the Office of Research Integrity aims to make additional updates to the UNE IRB website in an effort to continually improve the content available to the research community.<\/p>\n\n\n\n<p><strong>Prior to submitting your IRB application to <a href=\"mailto:irb@une.edu\">irb@une.edu<\/a> for review, please visit the UNE IRB website to ensure that you are using the most current version of any IRB-required documents
