The CEPH Chronic Disease Evaluation Team published two infographics highlighting evaluation results of the statewide diabetes and cardiovascular disease prevention and control programs managed by the Maine CDC to strengthen services, improve access, and increase reach. The infographics include how federal funds are being used to address health disparities through expanded programming and enhanced referral systems.
